Hooligan Office Reopening

Hooligan COVID-19 Safety Guidelines

The managers at Hooligan want you to know that your health and wellbeing is our top priority. To that end, we request that you read the following safety guidelines and watch the videos to ensure your safety in our office. All staff and visitors must review this material and provide contact information before arriving at Hooligan.

By checking ‘submit’ below I agree to the following:

Attending Hooligan Premises *NEW April 1,2021

  • I have been fully vaccinated (for more than two weeks since the second dose in a 2 dose series and (or) for more than 2 weeks since receiving one dose of a single dose vaccine), I agree that I will:

    ·    Provide evidence and date of vaccination

    ·    Understand that I am exempt from Quarantine rulings, but will continue to adhere to the request of the Managers at Hooligan with regard to wearing PPE, general hygiene practices and social distancing.

    ·    I will check in to the Hooligan office immediately after arrival and check out of the Hooligan office immediately before departure.

     

    I have not been vaccinated and have travelled outside of NYS to another U.S. State or Territory, I agree that I will:

    ·    I have received a negative COVID-19 test four days after returning to NY.

    ·    Continue to monitor for the onset of symptoms for 14 days.

    ·    Continue strict adherence to observe all practices of hand hygiene, and will wear a mask at all times.

    ·    Isolate if I experience any symptoms and contact the Managers at Hooligan as well as the local public health authority and immediately get tested.

    ·    I will check in to the Hooligan office immediately after arrival and check out of the Hooligan office immediately before departure.

 

Social Distancing

  • I will not be closer than 6 feet to any visitors or staff members and will wear a mask in the presence of other people, at all times.

  • I will not enter the designated common areas, which include the kitchen, lounge and café area if they are already occupied.

  • I will not enter any edit rooms that have already reached capacity.

Personal Protective Equipment

  • I will have quick access to a mask at all times, in the event that a staff member or visitor enters the same room as me.

  • I will not share my mask with anybody.

  • I understand the fundamentals of hygiene as well as the use and maintenance of PPE, and to this end, I have reviewed any information provided, including videos.

COVID-19 Contraction and Symptoms

  • I do not live in the same household with, or have had close contact with, someone who in the past 14 days was diagnosed with COVID-19, or had a test confirming they have the virus.

  • I have not been diagnosed with COVID-19 or had a test confirming I have the virus, within the last 10 days.

  • I have not had any one or more of these symptoms today or within the past 24 hours, which is new or not explained by a pre-existing condition:

    • Fever of 100.4˚ or greater, Chills, or Repeated Shaking/Shivering

    • Cough

    • Sore Throat

    • Shortness of Breath, Difficulty Breathing

    • Feeling Unusually Weak or Fatigued

    • Loss of Taste or Smell

    • Muscle Pain

    • Headache

    • Runny or Congested Nose

    • Diarrhea 

 

Agreement

  • Upon arrival at the Hooligan office, you will be required to agree to the safety guidelines, as stated above, and will provide contact information upon check in.

  • Remember, if you are sick or are exhibiting any of the above symptoms of COVID-19 (or if someone you live with has been lab-confirmed diagnosed with COVID-19) within the last 14 days, you must not report to work or attend sessions at the office.

  • I agree to notify Hooligan management if there are any changes to my circumstances that might influence the answers found in this document.
